- Please fill out the referral card using your computer for everything but the lower part of the form; then print it out, complete the last section and give it to your patient to bring with them to their first appointment with us. You will need

- 3D viewing software (if your computer can render the 3D images)
- Simple viewing software (for slower computers, no 3D image)
